How a new house design is helping Tanzanian children fight Malaria

In the rural villages of southern Tanzania, a simple change in the way houses are built is transforming the lives of children and giving families new hope in the fight against disease.

A major international study conducted in Mtwara Region has found that children living in specially designed “Star Homes” suffered far fewer cases of malaria, diarrhoea and respiratory infections compared to children living in traditional mud-and-thatch houses.

The three-year study, published in the respected medical journal Nature Medicine, is now being described as one of the most important public health breakthroughs linking housing and disease prevention in Africa.

Researchers found that children living in the Star Homes experienced 44 percent fewer malaria cases, 30 percent less diarrhoea and 18 percent fewer respiratory infections. 

The study also showed that younger children living in the homes grew taller and healthier, a sign that repeated illness and poor nutrition had reduced significantly.

For many families in Mtwara, the project has brought visible changes to daily life.

Children who once suffered repeated fever and illness are now spending more time in school and less time in clinics. Parents are also saving money that would otherwise be spent on medical treatment.

The Star Homes were designed to tackle several health problems at the same time.

Unlike ordinary rural houses, the structures have two floors, with bedrooms placed upstairs where mosquitoes are less common. The homes also include screened windows, self-closing doors, improved ventilation systems and shade-net walls that allow air to circulate while keeping insects out.

Other features include rainwater harvesting systems, smoke-free cooking stoves, fly-proof toilets and solar-powered lighting.

Professor Lorenz von Seidlein from the Mahidol-Oxford Tropical Medicine Research Unit said the project proves that smarter housing design can become a powerful tool in protecting children from disease.

“Malaria, diarrhoea, and respiratory infections have been killing children in sub-Saharan Africa for generations,” he said.

“Our results show that thoughtful house design insect-proof, smoke-free, cooler, and with clean water can protect children from all three at once.”

He added that the project should encourage governments and builders to rethink the future of rural housing.

“The Star Home may not be the final word, but it proves the concept and we hope it will inspire architects, builders, and governments to think differently about rural housing,” he said.

The study was conducted between January 2022 and December 2024 across 70 villages in Mtwara, one of Tanzania’s highest malaria burden regions.

More than 14,600 households were surveyed before families were selected through a public lottery. Researchers then compared the health of children living in 110 Star Homes with those living in 513 traditional houses.

During the three years, trained field workers visited households every week to monitor children’s health conditions.

For Salum Mshamu, the lead field investigator from CSK Research Solutions in Mtwara, the strongest evidence of success was not only the reduction in disease but also the improvement in children’s growth.

“The growth data may be the most striking finding,” he said.

“Children growing up healthier are the ultimate measure of success.”

He explained that repeated infections often damage a child’s development and future opportunities.

“Reducing stunting has lifelong consequences for education, earnings, and wellbeing — and we achieved it simply by building a better house,” Mshamu said.

Health experts say malaria, diarrhoea and respiratory infections remain among the biggest killers of children in sub-Saharan Africa.

According to the study, malaria alone caused an estimated 610,000 deaths globally in 2024, with Africa accounting for most of the cases and deaths.

Researchers involved in the project say the Star Home is important because it offers a practical and long-term solution that goes beyond medicine.

Architect and Dean of the Royal Danish Academy, Jakob Brandtberg Knudsen, said the project broke new ground by combining architecture and medical science in one large clinical trial.

“What is groundbreaking about the project is that architecture is incorporated as an active component of a randomized clinical trial and measured with the same level of rigour as medical research,” he said.

Knudsen said the success of the project was driven by cooperation between experts from different fields, including architects, doctors, scientists and social researchers.

“Architecture can function as a health intervention on a par with medicine when it is developed and documented using scientific methods,” he added.

Beyond health benefits, the homes were also found to be cheaper and more environmentally friendly.

Researchers say the Star Homes used 73 percent less concrete and produced 57 percent lower carbon emissions than conventional cement-block houses. Material costs were also 24 percent lower than ordinary modern homes.

Professor Steve Lindsay from Durham University said the findings could influence future housing development across Africa.

“Our ground-breaking study shows that building better can turn a dangerous home into a safe one by protecting children in sub-Saharan Africa against three major killer diseases,” he said.

Lindsay noted that the healthy house model was developed after decades of research into disease prevention and climate-friendly housing.

“We now hope that the building industry will adopt some of the important features of our healthy house design that are protective, cheaper and use less carbon than conventional concrete African houses,” he said.

Researchers say the project comes at a critical time as Africa’s population continues to grow rapidly, increasing demand for millions of new homes in coming decades.

The scientists believe that by including disease-reducing features in future housing projects, governments and private developers could make major progress in improving public health without increasing construction costs.

For families in Mtwara, however, the impact is already personal and immediate.

Children are falling sick less often. Parents are worrying less about hospital bills. Homes are becoming safer places to sleep.

And in communities where malaria once shaped everyday life, a new kind of house is now helping children grow stronger, healthier and more hopeful about the future.
